The following forms, created by either the ELCA or the Pacifica Synod, allow rostered leaders to report on ministerial, financial and statistical areas of their ministry. Click the link provided to download a PDF version of the forms or to connect with the ELCA website.

ELCA Reporting Forms

Annual Reporting for Rostered Ministers (Link to ELCA Website)

All rostered ministers in the ELCA are required to complete an annual report. Follow this link to select the applicable report. This report is a fillable PDF. To ensure that your report is saved properly before sending or printing it, please follow the instructions below.

Once completed, please return this form to the synod office by email (see below) or by mail to the Pacifica Synod office at 1801 Park Court Place, Bldg. C, Santa Ana, CA, 92701.

Right-click or control-click (on a Mac) on the file you want to use and select “save file” so it gets saved on your computer in a place you can find.

Close the webpage! Don’t type your answers in the webpage version! Find the file on your computer, and open it in Acrobat or Preview or another .pdf reader. Fill out a couple lines of the form. Save it. Close the file. Reopen it to see that the data is being saved correctly. Finish completing the file and save it.

Congregation Reports A (Link to ELCA Website)

On December 9, the ELCA churchwide organization sent out an email to ELCA congregations, SAWCs, and new starts, describing the Annual Congregation Report process and key dates for the new data year. The email included a link to elca.org/reportinstructions which includes a generic version of Form A, so congregations that wish to begin compiling their data can do so.

The online system for 2025 Form A reporting will open in mid-January. It will be accessible at elca.org/congregationalreport. All ELCA congregations, SAWCs and new starts will receive the 2025 Annual Congregation Report letter through the mail at this time.

These passwords will be provided to congregations and worshiping communities in a letter from Secretary Rothmeyer that will be sent through the mail. The letter should arrive mid-January, so keep your eyes open for it.

2026 Mission Support Statement of Intent

This annual report created by the ELCA helps the synod in their budget planning. Please submit your Plan for Mission Support to the synod office as soon as you are able. Click here for a fillable form in English. Once completed, please return this fillable form by email to Terri Robertson at  terrirobertson@pacificasynod.org or by mail to the Pacifica Synod office at 1801 Park Court Place, Bldg. Pacifica Synod Expense Reimbursement Forms

2025 Pacifica Synod General Expense Form (PDF)

Those submitting expenses to the synod for reimbursement (excluding Hawaii Synod Assembly travel) are asked to complete and return this form, along with copies or originals of your receipts, to the Pacifica Synod bookkeeper (terrirobertson@pacificasynod.org) for reimbursement.

Hawaii Synod Assembly Travel Equalization Form (PDF)

This form is to be used by Hawaii Synod Assembly voting members to request a reimbursement of their airfare to and from the mainland for the annual Pacifica Synod Assembly. The maximum reimbursement amount is $450 per voting member. Receipts must be provided.
